Affidavit of Subscribing Witness (Self-Proving)

A pre-prepared witness affidavit attached to a self-proving will. If the will includes this, the witness may not need to appear in court.

This form must match exactly what's in the will. If the affidavit doesn't align with the will's execution details, the court may still require the witness to appear in person.
